Case Manager Registered Nurse (RN)

Piper Companies is seeking a Case Manager Registered Nurse (RN) to join a leading organization in the health insurance industry for a fully remote position based out of NC or other eligible compact states. The Case Manager Registered Nurse (RN) will support members through comprehensive telephonic case management and coordinated care planning to achieve optimal clinical outcomes.

Responsibilities of the Case Manager Registered Nurse include:

· Conduct comprehensive telephonic assessments and develop individualized, member‑centric care plans.

· Coordinate care needs across multidisciplinary teams, community resources, and provider networks.

· Perform ongoing monitoring through scheduled follow‑ups, goal review, and care plan adjustments based on member acuity.

· Evaluate appropriateness of services across the continuum of care and support transitions between settings.

· Document all assessments, interventions, progress notes, and outcomes in compliance with internal standards.

Qualifications for the Case Manager Registered Nurse include:

· RN with at least 3 years of clinical and/or case management experience, including telephonic case management.

· Valid and active RN license in NC or compact multi‑state licensure.

· Experience supporting diverse case types and collaborating across provider and resource networks.

· Strong assessment skills with ability to identify barriers, educate members, and coordinate services.

· Additional specialty certifications such as CCM or CDCES preferred or required depending on role (must be obtained within 2 years if required).

Compensation for the Case Manager Registered Nurse:

· Salary Range: $68,000-$78,000/year (USD)

· Comprehensive Benefits: Medical, Dental, Vision, sick leave if required by law, and 401K
