Position Overview

We are seeking a highly motivated Clinical Pharmacist to support Medicare quality initiatives, with a focus on improving medication adherence and closing care gaps—particularly SUPD (Statin Use in Persons with Diabetes).

In this role, you will engage directly with Medicare members, providers, and pharmacies to deliver high-quality, patient-centered care and ensure optimal medication outcomes. You will also conduct Comprehensive Medication Reviews (CMRs) to enhance members’ understanding of their treatment plans and improve overall health outcomes.

Key Responsibilities

  • Collaborate with pharmacies and providers to resolve adherence gaps when members are unengaged or need additional support
  • Identify and close SUPD (Statin Use in Persons with Diabetes) gaps through provider engagement
  • Communicate with providers by fax or CMR platforms to recommend interventions (e.g., drug interactions, dose adjustments, adherence concerns)
  • Provide medication counseling and education to Medicare members
  • Educate members on pharmacy benefits, including formularies, preferred pharmacies, and supplemental benefits
  • Support Pharmacy Quality Specialists with clinical guidance and expertise.
  • Maintain accurate documentation and ensure compliance with quality and regulatory standards

Qualifications

Required:

  • Bachelor’s degree in Pharmacy or advanced degree (PharmD preferred)
  • Active North Carolina pharmacist license and residency
  • Minimum 3+ years of experience in a related field
  • Strong knowledge of medication therapy management and adherence strategies

Preferred:

  • Experience with Medicare, Medicaid, or managed care environments
  • Familiarity with MTM/CMR programs and Medicare STARs metrics
  • Background in retail pharmacy, health plans, or DSNP/HMO populations

Core Competencies

  • Strong patient communication and counseling skills
  • Ability to manage high-volume outreach (phone-based engagement)
  • Clinical knowledge of drug therapies, interactions, and disease states
  • Detail-oriented with strong organizational and documentation skills
  • Ability to work independently in a remote, fast-paced environment
  • Background in retail pharmacy, health plans, or DSNP/HMO populations

Compensation

Salary Range: $110,000 - $125,000 with company subsidized medical, dental, and vision benefits
