Piper Companies is seeking a Case Manager to support a leading organization within the health insurance and managed care industry. This fully remote Case Manager opportunity is available to candidates located in North Carolina and offers the chance to collaborate with interdisciplinary healthcare teams to improve member outcomes, coordinate services, and promote quality, cost-effective care.

Responsibilities

  • Conduct comprehensive evaluations to identify member needs, risks, goals, and support requirements, creating individualized care strategies.
  • Partner with members, families, healthcare providers, and community organizations to coordinate services across the healthcare continuum.
  • Monitor member progress through regular outreach, reassessments, and care plan updates to ensure optimal clinical and resource utilization outcomes.
  • Provide education, advocacy, and guidance to encourage preventive care, treatment adherence, and informed healthcare decisions.
  • Maintain accurate documentation of assessments, interventions, care coordination activities, and member outcomes while ensuring regulatory and organizational compliance.

Qualifications

  • Active RN, RD, or LCSW license with a minimum of three years of clinical, care coordination, or case management experience required.
  • Master's in Social Work with five years of related experience may be considered for select positions.
  • Behavioral Health-focused candidates may qualify with alternative applicable licensure and at least three years of direct clinical experience.
  • Must possess and maintain a valid North Carolina or compact multi-state clinical license in good standing.
  • Strong assessment, communication, care planning, documentation, and interdisciplinary collaboration skills required.

Compensation

  • Salary Range: $66,000 - $80,000 annually
  • Comprehensive Benefits: Medical, Dental, Vision, sick leave if required by law, and 401K
