JOB DESCRIPTION

Piper Clinical Solutions is seeking a motivated Care Coordinator Nurse in Anne Arundel County, MD to support a local health system with utilization management, discharge planning, and coordination activities in an inpatient facility.

Responsibilities for the Care Coordinator Nurse

  • Conducting assessments and patient care coordination to achieve successful outcomes for acute care patients
  • Evaluate patient care needs to develop plan of care and timely discharge
  • Complete review of patient medical records and provided services to facilitate ongoing patient care coordination
  • Identify and develop discharge planning needs and determine potential obstacles for discharge
  • Collaborates with physicians, nurses, and other staff involved with care of the patient to develop a coordinated approach to each patient’s care
  • Communicate with physicians to determine plan of care, anticipated discharge, and additional outpatient services to avoid delays in patient care
  • Collaborate with Utilization Management Nurses to assess patient's clinical progression and validate need for acute hospital level care
  • Intervene with medical staff concerning delays in service that affect quality of care or length of stay
  • Collaborate with Utilization Review Nurses to detect factors contributing to delays in service or delayed discharge
  • Prepare patients and families for transition from inpatient to outpatient or alternative setting
  • Involve patients and families in their discharge planning to create realistic expectations for post-hospital care
  • Educate patients and families on illness/disability and capacity
  • Maintain patient record documentation to reflect plan of care to address post-hospital care needs and evidence of patient/family involvement in planning
  • Develop and arrange discharge plans for patients discharged
  • Refers patients with complex discharge or psychosocial needs to Social Workers and communicates with Placement Specialists to ensure safe transitions to lower levels of care

 

Requirements for the Care Coordinator Nurse

  • 5+ years of Nursing Experience
  • Experience with Case Management, Discharge Planning, or Utilization Review/Management
  • Current Maryland RN License

 

Compensation for the Care Coordinator Nurse

  • Salary Range: $70,000 – $95,000 depending on years of experience and meeting of requirements
