JOB DESCRIPTION

Piper Clinical Solutions is seeking a motivated Local Care Coordinator Nurses in Washington, D.C. & Silver Spring, MD to support a local health company with member care plan development, discharge planning, and patient services coordination activities.

Responsibilities for the Local Care Coordinator Nurse:

  • Provide on-site consultation to providers and patients related to the implementation of appropriate care plans
  • Develop documentation for the Care Plans for any assigned patients
  • Track patient progress, self-management support, implementation of care plans and identify any work process or patient flow improvement areas to ensure the completion of coordinated services for patients
  • Facilitate and monitor the transition of care for assigned patients
  • document patient progress toward established health state goal and any barriers to achievement of Care Plan outcomes
  • Develop communication and referral processes to ensure seamless communication between providers, patients and the Care Coordination Team
  • Work with Regional Care Directors and Practice Consultants to develop clinical reports for use in provider offices to support patient care coordination
  • Assist patients with coordination of any additional care needs and consult with specialists when needed
  • Perform comprehensive medication reconciliation (CMR) at the onset of the Care Plan and every 30 days thereafter during the Care Plan lifecycle
  • Assess patient’s ongoing care needs and progress towards goals throughout the case duration and makes revisions as needed to address changes in patient condition, lack of response to the care plan, preference changes, and transitions in care settings
  • Coordinate plan of care with the provider with goals of member stabilization, decreased admissions and medication management
  • Coordinate patient education in support of standards of care guidelines and related health issues

 

Qualifications for the Local Care Coordinator Nurse:

  • 3+ years clinical acute care Nursing experience
  • Utilize databases, spreadsheets, Microsoft office, and other technologies in effective manner
  • Active RN License in Maryland AND D.C.
  • Ability to travel 50% (variable) by own automobile to assigned local practices (Remainder of hours worked are remotely based)

 

Compensation for the Local Care Coordinator Nurse:

  • Salary up to 86,000 based on meeting of requirements
  • Bonus incentive plan up to $6,000 annually w/ additional target metrics bonuses up to $1300 every month
